The questionnaire is only 4 pages; Fast and convenient for your clients to complete, yet providing you with valuable feedback. There is a crucial balance in the number of questions you want to ask and the time it takes your client to complete a survey. With 19 total questions divided over 4 pages, our research indicates we have just the right balance. Most surveys can be completed in less than 3 minutes – much faster than with paper and postage.

Page 1 – This page determines your overall funeral home rating. Answers to these 5 questions are used to determine the performance rating of each funeral home and are displayed on the individual rating page of subscriber funeral homes.
Clients can also add comments at any time during the survey.

Page 2 – Customized to the needs of each funeral home, you may ask up to 7 questions specific to your needs. More than 12 example questions are provided to assist you or you may design your own questions when setting up your account.
Page 3 - Profile your employees to be rated individually! Your clients can rate funeral directors, arrangers and pre-need counselors employed by your funeral home. In addition to the 3 questions displayed on their rating profile, you may ask up to 2 additional questions (optional) for internal use and reporting.

Page 4 – Capture marketing data on this last page with the 2 questions that are vital to your marketing decisions.
